Go to Mail-order Form, fill in boxes, and print form. |

Send check or money for $6.50 along with Order Form. |

If you are not able to print the Order Form, just write the needed information from the Order Form onto a blank sheet of paper. |

When will I receive my order |

Whether you pay by mail or by PayPay, the cost of shipping and handling is included. Please include a telephone number or an e-mail address, in case there is a need for us to receive clarification from you. You can expect to receive labels within 5 to 10 business days of our receipt of order. |
